月度体育活动的参赛资格 / Eligibility for Monthly Sports Activities

月度体育活动仅向DUSA会员、DUSA会员单位的员工或经DUSA确认的会员单位授权代表开放。除非DUSA事先书面同意，任何非DUSA会员、非会员单位员工、朋友、家属、客户、供应商、临时替补或其他第三方均不得参加月度体育活动，亦不得进入活动场地参与比赛、训练、热身或其他体育活动。

Sports Circle-Monthly sports activities are open only to DUSA members, employees of DUSA member companies, or authorized representatives of member companies confirmed by DUSA. Unless DUSA gives prior written consent, no non-DUSA member, non-member company employee, friend, family member, customer, supplier, temporary substitute or other third party may participate in any monthly sports activity or enter the activity venue for any match, training, warm-up or other sports activity.

个人确认原则 / Personal Acknowledgement Requirement

每一名实际参赛者均应以本人名义完成报名、签到、电子确认或书面签署，并亲自阅读、理解并同意DUSA的活动规则、风险告知、健康声明、紧急救助授权及责任限制条款。未经本人完成确认的人员，不得参加活动或进入活动场地参与比赛、训练、热身或其他体育活动。

Each actual Participant shall complete registration, check-in, electronic acknowledgement or written signing in his/her own name, and shall personally read, understand and agree to DUSA's activity rules, risk disclosure, health declaration, emergency assistance authorization and limitation of liability provisions. Any person who has not personally completed such acknowledgement shall not participate in the activity or enter the activity venue for any match, training, warm-up or other sports activity.

自愿参加及风险知悉 / Voluntary Participation and Acknowledgement of Risks

参赛者确认，其系自愿报名并参加由DUSA组织、协助、支持或认可的相关体育、社交或会员活动。参赛者理解并确认，体育活动具有一定固有风险，包括但不限于身体碰撞、摔倒、运动损伤、心脑血管事件、突发疾病、昏迷、心脏骤停、身体伤害乃至死亡等风险。

The Participant confirms that he/she voluntarily registers for and participates in the relevant sports, social or member activities organized, assisted, supported or recognized by DUSA. The Participant understands and acknowledges that sports activities involve inherent risks, including but not limited to physical contact, falls, sports injuries, cardiovascular or cerebrovascular events, sudden illness, unconsciousness, cardiac arrest, bodily injury and even death.

 健康声明及自我评估义务 / Health Declaration and Self-assessment Obligation

参赛者声明，其已根据自身年龄、身体状况、既往病史、运动能力及医生建议，审慎判断自身是否适合参加相关活动。参赛者承诺，如其患有或曾患有心脏病、高血压、脑血管疾病、呼吸系统疾病、癫痫、严重过敏、近期重大手术或其他可能影响参加体育活动安全的疾病或身体状况，应在参加活动前自行咨询医生意见，并审慎决定是否参加。

The Participant declares that he/she has carefully assessed whether he/she is fit to participate in the relevant activity based on his/her age, physical condition, medical history, athletic ability and medical advice. The Participant undertakes that, if he/she has or has had any heart disease, hypertension, cerebrovascular disease, respiratory disease, epilepsy, severe allergy, recent major surgery or any other disease or physical condition that may affect the safety of participation in sports activities, he/she shall seek medical advice before participating and carefully decide whether to participate.

身体不适时停止参加 / Stopping Participation When Feeling Unwell

参赛者进一步承诺，如在活动前或活动过程中出现胸闷、胸痛、头晕、呼吸困难、心悸、明显疲劳、意识模糊、肢体无力或其他身体不适，应立即停止参加活动并及时告知现场组织人员。参赛者不得隐瞒可能影响其安全参加活动的身体状况。

The Participant further undertakes that, if he/she experiences chest tightness, chest pain, dizziness, breathing difficulty, palpitations, obvious fatigue, confusion, limb weakness or any other physical discomfort before or during the activity, he/she shall immediately stop participating and promptly inform the on-site organizers. The Participant shall not conceal any physical condition that may affect his/her safe participation in the activity.

风险自担 / Assumption of Risk

在适用法律允许的范围内，参赛者同意自行承担因其自身健康状况、既往疾病、隐性疾病、身体原因、未充分评估自身身体状况或体育活动固有风险所导致的人身损害、财产损失或其他不利后果。除非DUSA、活动组织人员、其他参赛者、志愿者或相关人员存在故意、重大过失或依法不得免责的情形，参赛者不得就上述风险后果向DUSA、活动组织人员、其他参赛者、志愿者或相关人员提出索赔。

To the extent permitted by applicable law, the Participant agrees to assume responsibility for any personal injury, property loss or other adverse consequences arising from his/her own health condition, medical history, latent disease, physical condition, failure to adequately assess his/her own physical condition, or the inherent risks of sports activities. Unless DUSA, the activity organizers, other participants, volunteers or relevant persons have acted with intent, gross negligence, or under circumstances where liability cannot be excluded by law, the Participant shall not bring any claim against DUSA, the activity organizers, other participants, volunteers or relevant persons in respect of the above risks and consequences.

合理安全保障义务 / Reasonable Safety Protection Obligations

DUSA将在与活动性质、参与人数、场地条件及风险水平相匹配的合理范围内，提供必要的活动组织、风险提示和应急协助安排。参赛者理解并确认，DUSA的上述安排不构成对参赛者身体状况、运动能力、参赛安全或活动结果的任何保证。

DUSA shall, within a reasonable scope proportionate to the nature of the activity, the number of participants, venue conditions and risk level, provide necessary activity organization, risk warnings and emergency assistance arrangements. The Participant understands and acknowledges that such arrangements do not constitute any guarantee by DUSA of the Participant's physical condition, athletic ability, participation safety or activity outcome.

月度体育活动（无保险安排） / Monthly Sports Activities (No Insurance Arranged)

DUSA通常不为月度体育活动（Sports Circle）统一购买保险。参赛者理解并确认，其系自愿参加月度体育活动，并自行承担相应风险；DUSA建议参赛者根据自身情况自行投保人身意外保险。DUSA未安排保险，不影响本条款项下风险自担及责任限制的约定，亦不减轻参赛者应尽的注意义务。

DUSA generally does not arrange unified insurance for monthly sports activities (Sports Circle). The Participant understands and acknowledges that he/she participates in monthly sports activities voluntarily and at his/her own risk, and DUSA recommends that the Participant arrange his/her own personal accident insurance as appropriate. The absence of insurance arranged by DUSA shall not affect the assumption of risk and limitation of liability under these Terms, nor mitigate the Participant's own duty of care.
